Hardware Transfer of Ownership

By submitting this request, the new owner represents that they are the current owner of the equipment referenced above and that such equipment was acquired directly from the former owner and that they have the former owner's permission to transfer ownership to the new owner. DEXIS reserves the right to deny any requested transfer of ownership. Any transfer of ownership is subject to the terms and conditions of the DEXIS transfer of ownership of DEXIS imaging equipment terms and conditions.

A DEXIS Account Manager will contact the new owner to discuss the extended warranty if one is attached to the equipment.
